This HIPAA Privacy Policy explains how DermaGlow Lab & Aesthetics by Tatarian, MD, a California professional medical corporation (“we,” “us,” or “our”), may use and disclose your Protected Health Information (PHI) and describes your rights regarding that information.   • For most disclosures of psychotherapy notes (if applicable)

You have the legal right to:

  • Request Restrictions on the use or disclosure of your PHI
  • Receive Confidential Communications in a preferred manner or location (e.g., via mail to your home or work)
  • Inspect and Copy your health records (fees may apply)
  • Request an Amendment to your health records if you believe they are inaccurate or incomplete
  • Receive an Accounting of Disclosures of your PHI made in the past six years (excluding those for treatment, payment, and healthcare operations)
  • Request a Paper Copy of this policy, even if you received it electronically

Safeguarding Your Information

DermaGlow Lab & Aesthetics uses industry-standard administrative, physical, and technical safeguards to protect your PHI against unauthorized access, loss, or disclosure. These include encrypted systems, access control measures, and privacy protocols for staff.

Changes to This HIPAA Privacy Policy

We reserve the right to change this policy at any time. Changes will apply to all PHI maintained by our practice. If significant updates are made, we will post a revised policy on our website and/or display it at our facility. The effective date will be updated accordingly.
